Assessment of Serum Level of Thymic Stromal Lymphopoietin (TSLP) in Patients with Acne Vulgaris

Abstract

Background: Disrupted sebaceous gland function, increased sebum production, and sebum fatty acid composition contribute to acne. Thymic stromal lymphopoietin (TSLP) boosts type 2 immunological responses and induces fat loss by activating sebaceous glands to secrete sebum, which strengthens the skin barrier. Objective: to compare serum TSLP levels in acne vulgaris (AV) patients to normal controls and to determine if TSLP levels affect acne severity. Methods: Between June 2023 and December 2023, 30 patients and 30 healthy controls visited the dermatological outpatient clinic at Beni-Suef University. Results: The levels of TSLP in the blood of patients were noticeably greater than those of controls (589.09±235.31 vs 172.91±38.36 pg/ml; P<0.001). In instances that were analyzed, there was a strong positive correlation between TSLP and both the severity score (r=0.575; P=0.001*) and the duration of the disease (r=0.801; P<0.001*). An increased TSLP was linked to a more severe disease score (P<0.001) and the development of chronic illness (P=0.014). AUC 0.973 (95%CI: 0.922-1.0; P <0.001), 96.7% sensitivity, 96.7% specificity, 96.7% PPV, 96.7% NPV, and 96.7% accuracy were shown by the ROC curve for TSLP when distinguishing cases from the control group at a cutoff value of 230.55 pg/ml. Conclusion: TSLP serum levels were substantially greater in AV patients than healthy persons. In analyzed situations, TSLP strongly linked with disease duration and severity. Chronic illness start and severe disease score were strongly linked with higher TSLP.
